SCHOOL INSTRUMENT RENTAL AGREEMENT WITH PURCHASE OPTION I understand that upon submitting this form, Eastside Music Ltd. will charge my credit card to cover the cost of rental and insurance for the first three months on the requested instrument.

Monthly after initial period: $40.00
Maintenance/Insurance: $5.00 (included in monthly fee)
GROUP B INSTRUMENTS
Alto Saxophone, French Horn, Cello
Initial Amount: $195.00
Monthly after initial period: $75.00
Maintenance/Insurance: $6.00 (included in monthly fee)
I understand that the cost of the method book and/or music stand will also be added to the initial rental fee if I selected those options and that the cost of the book (chosen by my school district) can range from $9.00 to $15.00 and the music stand will cost $25.00.
I understand that on the expiration date of stated rental period it is my responsibility to return the instrument directly to Eastside Music Ltd. in the same condition as when received, with only reasonable wear and tear resulting from proper use of the instrument being acceptable.
Special arrangements must be made if the instrument is to be returned through the school. After the initial rental period, I understand that my credit card will continue to be charged on a monthly basis at the agreed upon rental rate until the instrument is paid in full or returned.
I understand that the monthly rental rate as described above includes a maintenance/insurance fee which does not apply toward the purchase price of the instrument. A late charge of $2.00 will be added to all accounts 5 days past due.
Maintenance and insurance is only in effect as long as payments are current.
Eastside Music Ltd. retains the full title and ownership of the instrument until full payment has been made to the balance of the instrument price. I agree not to sell, mortgage, or dispose of said property until such payment is made. Where non-payment of contract exists for 30 days or longer, Eastside Music Ltd. reserves the right to bill the rental and any late fees or NSF check charges, plus 10% of the transaction on a valid credit card. Eastside Music Ltd. reserves the right to pick up instruments anytime or anywhere for non-payment of delinquency of rental.
I understand that if I have a change of address without notifying Eastside Music Ltd., they have the right to bill the entire contract balance on a valid credit card. Instruments paid in full within the first year of the contract will receive a 20% discount from the current balance.
Note to customers renting violin, viola, or cello: In the event it should be necessary to exchange for a larger size instrument, a maximum of 75% of the rental paid on the smaller instrument will apply to the larger size.
INITIAL RENTAL PERIOD BEGINS ON THE DATE THE INSTRUMENT IS DELIVERED. YOUR INITIAL RENTAL PERIOD ENDS AFTER THE FIRST THREE MONTHS. YOUR FIRST REGULAR MONTHLY RENTAL PAYMENT WILL BE CHARGED TO YOUR CREDIT CARD IMMEDIATELY FOLLOWING THE INITIAL THREE MONTH RENTAL PERIOD.
MAINTENANCE COVERAGE AND EXCLUSION This includes any and all repairs that result from normal everyday use of the instrument; i.e. key adjustment, pad replacement, or key corks. Regretfully, dents, seized slides, and bent keys do NOT occur during proper playing or maintenance of the instrument and will therefore be excluded from coverage. All instruments will be inspected upon return and any charges for damage will be billed to your credit card.
Damage incurred by anyone attempting to repair the instrument, other than an Eastside Music technician, is excluded.
Replacement of accessories is excluded. You are responsible for accessories; i.e. reeds, lyres, neck screws, lyre screws, finger rings, cleaning equipment, mouthpieces, drumsticks, and/or mallets.
Insurance is for fire and theft only. Eastside Music Ltd. will replace an instrument upon receipt of a fire or police report. An instrument in covered for service or insurance only if payment is kept current. A lapse in payment means a lapse in coverage. You are responsible for damage to the instrument.
Renter agrees to immediately notify Eastside Music Ltd. of any changes in address or telephone number.

DO NOT leave the instrument at your child's school unless special arrangements have been made with Eastside Music to pick up the instrument. Renter is responsible for payment until paid in full or returned to Eastside Music. If instrument received at time of initial rental is not a new instrument, renter may only exchange for a new instrument within the first year of the contract. Any exchanges must be made within the first year of contract. It is the responsibility of the renter to request a new instrument. You will not be given a new instrument automatically. A new instrument cannot be exchanged for another new instrument (unless they are different types of instrument, i.e. saxophone for flute). A maximum of three exchanges are allowed in the first year of the contract. Renter understands that the cost of a new instrument is greater than that of a used instrument.
DEFAULT THIS IS NOT A PURCHASE AGREEMENT, it is a rent-to-own agreement. The renter agrees that he/she has rented the items hereto described, upon the express condition that it will at all times REMAIN THE PROPERTY OF EASTSIDE MUSIC LTD. until it is paid in full. The renter agrees that he/she will pay promptly, when due, all charges which accrue during the rental period. In the event that the renter fails to make monthly payments or fails to return the instrument directly to Eastside Music Ltd., we reserve the right to repossess the instrument without notice or to demand immediate payment of the current balance in full. Any unpaid late charges that accrue will be billed to your credit card.
Any denied credit card charges or checks returned to us, UNPAID, will be subject to a $15.00 charge.
